             <title>前列腺纤维化</title>

                 前列腺纤维化多发生于老年人，是慢性前列腺炎或老年性退行性改变的结果。前列腺腺体萎缩，体积变小，质地变硬。轻者无临床症状，重者有时引起排尿困难。一般不发生血尿。超声显像可以方便地发现前列腺纤维化，对老年人排尿困难的原因作出鉴别。声像图显示前列腺体积明显缩小，包膜完整。内部回声增强，不均匀，常可见到不规则的点状强回声。 <BR><BR>                 ]]>

             <title>前列腺所因起的阳萎</title>

                 随着生活水平的提高，对阳萎有治疗要求的人亦增多。竞争的加剧，阳萎的发病率亦增高。<br>
            　　阳萎指男子性交时阴茎不能勃起，或勃起硬度不足，或不持久（即易疲劳软），致使不能进行性交。<br>
            　　近几年来由于检查方法的不断改进，对阳萎的病因也有了进一步的认识，现已证明阳萎与以下几个方面有关： <br>
            　　1. 非器质病变 即大脑皮质对性兴奋的抑制作用加强，脊髓勃起中枢兴奋性减退。也就是所谓功能性原因，没有器质性病变存在，可以是长期手淫或纵欲过度，使神经系统经常处于过度兴奋状态，最终因兴奋过度而衰竭。也可以是慢性病、体质衰弱或过度疲劳，是体力或脑力劳动所引起的身体衰弱或神经衰弱所致。另一种原因是出自精神因素，例如害怕妊娠、性交环境不良、夫妇感情冷淡，或以为从前遗精、手淫过多而担心性功能有问题，甚至出于性知识缺乏而产生的恐惧或害怕女方责怪的心理所造成。<br>
            　　2. 器质性原因 因老年、各器官系统病变、药物影响或手术等所致的阳萎，这类病人即使在强力性刺激情况下阴茎都不能勃起，约占60%，归纳起来器质性阳萎的原因有：<br>
            　　（1）神经障碍：如脊髓损伤、脊髓横断、脊髓肿瘤、颞叶病变， 都可因传导性兴奋的神经障碍而引起阳萎。<br>
            　　（2）血运不足：动脉粥样硬化或其他血管病变均可导致血运不足。动脉粥样硬化，如发生于供应阴茎的动脉或支配营养的血管，亦可使病人发生阳萎。<br>
            　　（3）内分泌障碍：主要是糖尿病，下丘脑-垂体异常及原发性性腺功能不全：<br>
            　　① 糖尿病约占50%病人发生阳萎，较同龄正常的人群多2-5倍，如有外周及植物性神经的病变及血管异常更可加重阳萎，阳萎的程度一般与糖尿病血管及视网膜病变的程度并不一致。发生阳萎的因素主要由糖尿病造成的代谢紊乱神经病变；糖尿病引起的闭孔动脉狭窄所致的阴茎血流降低，以及糖尿病造成的代谢紊乱因素等；<br>
            　　② 下丘脑垂体异常，约占阳萎病人的7-19%，主要是垂体肿瘤，其他有周围病灶浸润或垂体血运障碍等。促进阳萎的因素为促进性腺激素释放激素（GnRH）减少导致黄体生成激素或促卵泡生产激素（LH及FSH）减少和催乳素增多；<br>
            　　③ 原发性性功能不全，常见的有先天性和后天性两种：前者如Klinefelter综合征，先天性双侧无睾症。后者如流行性腮腺炎并发睾丸炎、血管疾病、化疗或放疗等；<br>
            　　④ 皮质醇增多症、双侧肾上腺皮质增生、腺瘤、腺癌或医源性导致的皮质醇增多，可抑制促性腺激素及睾丸间质细胞分泌睾丸酮而引起阳萎；<br>
            　　⑤ 甲状腺功能亢进症，约有56%的甲状腺功能亢进者能发生阳萎，但机理尚不清楚；<br>
            　　⑥ 甲状腺功能减退，此种疾病造成甲状腺激素分泌减少，睾酮及睾酮结合球蛋白降低，催乳素增多。阳萎的发生原因在于全身蛋白合成障碍导致睾丸曲细精管退行性病变，及间质细胞减少有关；<br>
            　　⑦ 肾上腺功能不足， 是与消瘦、营养不良，以致垂体分泌LH及睾丸间质细胞分泌睾酮减少有关；⑧女性化肿瘤，此肿瘤可发生在肾上腺或睾丸间质细胞，实验室检查有雌激素增多改变，导致男性乳房发育，睾丸萎缩引起阳萎。<br>
            　　3、其他器质性疾病 <br>
            　　包 括：<br>
            　　① 药物影响，例如胍乙啶、利血平、甲基多巴、安宫黄体酮、抗胆碱能药物、雌激素等；<br>
            　　② 各种手术并发症，如经会阴前列腺切除术、交感神经切除术、直肠癌根治术，可影响勃起的解剖学和生理学而造成阳萎；<br>
            　　③ 毒性物质，例如铅和除莠剂（二种阻碍植物生长化学剂）；<br>
            　　④ 外伤，如睾丸外伤阉割、骨盆骨折、阴茎外伤等；<br>
            　　⑤ 泌尿系统疾病，阴茎硬结症、阴囊积水、精索静脉曲张、包茎、阴囊橡皮肿等都可能引起阳萎。<br>
            　　中医病因：<br>
            　　1、先天遗传，体质虚弱。<br>
            　　2、日常生活不注意保养，造成体虚。如食少，睡眠不足，过于疲劳，偏食，长期酗酒，工作时间过长。<br>
            　　3、情绪压抑，心情不舒畅，或夫妻关系差。<br>
            　　4、疾病后。如感冒后，腹泻后，其他大病后……等，都需要一定的恢复期。<br>
            　　5、药物因素：如长期服清热消类炎药物，或长时间使用抗生素等。<br>
            　　6、思虑过度，耗伤气血。<BR>

             <title>坐姿可改善前列腺增生</title>

                 前列腺增生患者不宜久坐，已为许多人所共识，可是，前列腺增生患者坐的姿势有讲究，则为人们所忽视。 <BR><BR>　　当人正常端坐的时候，重心自然落于前列腺的位置，坐的时间久了，增生的前列腺必然要承受体重的压力，因而难免造成增生的前列腺向尿道管扩张，而压迫尿道，严重者会造成排尿困难，甚至闭尿。如果前列腺增生患者日常坐的姿势有意识地将重心移向左臀部或右臀部（可以左右臀部适当轮换）这样，就可以避免人体重心直接压迫增生的前列腺，从而避免或减轻增生的前列腺向尿道压迫。长期采用此方法，对增生的前列腺无疑可以起到意想不到的保护作用。 <BR><BR>

             <title>“腺”没了，“爱”依旧</title>

<br>我患有良性前列腺增生，医生建议手术切除前列腺。我害怕术后性功能会完全丧失。请问，会发生这样的情况吗？

<br>当然，手术的恢复期，不同的手术方式，对性功能或多或少会有些影响，如经尿道的手术，部分人会出现逆行射精；耻骨上经膀胱的手术，多数人会发生逆行射精；经会阴部的手术，阳痿的发生率较高。
<br>
<br>不过，大多数人经过康复休养，都能恢复性功能，不会对夫妻生活造成明显影响。但个人的体质和手术情况各不相同，因此，恢复的时间长短不一，也不能保证每个人的性能力都能恢复到术前的水平。
<br>
<br>需要注意的是，采用经尿道或耻骨上经膀胱的方法，术后感觉“不行了”，大多数人是心理性的，只要解除心理的包袱，重获激情并不难。患者术后别刻意和从前相比，特别是在恢复期，如果性能力不如从前，比的结果只能使自己越来越泄气，从而陷入恶性循环中。
<br>
<br>同时，也不要太和高潮“较真”，有些患者在术后会出现“逆向射精”，影响了高潮时的快感，心理上感觉不舒服而影响性生活。还有些患者的伴侣，想当然地认为患者在术后性能力会下降，而对性生活没有兴趣，也是导致患者发生阳痿的原因之一。

<br>所以在术后，伴侣要先对性生活投入热情，来带动患者“进入角色”。
<br>
<br>此外，没了前列腺，精液的总量会因缺乏前列腺液而减少一些，但“性趣”并无两样。<BR>

                 年龄是前列腺增生发病的基本条件之一。40岁对于人的发育来说是个重要的转折点，正如《素问·阴阳应象大论》中所说：“年四十，而阴气自半矣，起居衰矣”。说明40岁以后，人的各组织器官在开始走下坡路，譬如人的前列腺组织中间质成分相对比上皮组织更活跃，发生前列腺增生时，主要表现为间质增生。<BR>　　<BR>　　 虽然人们对前列腺增生病因尚未彻底明了，但以下措施对减轻病情及推迟该病的发生仍有一定的价值。<BR>　　<BR>　　 1.饮食应以清淡、易消化者为佳，多吃蔬菜水果，少食辛辣刺激之品，戒酒，以减少前列腺充血的机会。<BR>　　<BR>　　 2.尽可能少骑自行车，因自行车座可压迫前列腺部位，加重病情。<BR>　　<BR>　　 3.切忌长时间憋尿，以免损害逼尿肌功能加重病情。<BR>　　<BR>　　 4.保持心情舒畅，避免忧思恼怒，切忌过度劳累。<BR>　　<BR>　　 5.适度进行体育活动，有助于增强机体抵抗力，并可改善前列腺局部的血液循环。<BR>　　<BR>　　 6.及时治疗泌尿生殖系统感染，积极预防尿潴留的发生。<BR>　　<BR>　　 7.对于性生活，既不纵欲，亦不禁欲，可据年龄和健康状况而定。<BR>

             <title>前列腺增生的食疗法</title>

                 中医历来对饮食在疾病的发生、发展、康复过程中所起的作用非常重视。对于前列腺增生症来说，膏粱厚味、辛辣甘甜，常易引起湿热内生、阻抑气血运行，导致湿热下注、血阏气阻。因此，患前列腺增生症的老人应注意饮食清淡，多食青菜水果，戒烟少酒，慎食辛辣，并保持大便通畅。此外，选用下述饮食疗法对前列腺的保健和前列腺增生的治疗有益。<BR><BR>　　1、参芪冬瓜汤：党参15克，黄芪20克，冬瓜50克，味精、香油、盐适量。将党参、黄芪置于砂锅内加水煎15分钟去渣留汁，乘热加入冬瓜至熟，再加调料即成，佐餐用。有健脾益气，升阳利尿之功效。<BR><BR>　　2、桂浆粥：肉桂5克，车前草30克，粳米50克，先煎肉桂、车前草，去渣取汁，再加入粳米煮熟后加适量红糖，空腹服。有温阳利水之功效。<BR><BR>　　3、杏梨石苇饮：苦杏仁10克，石苇12克，车前草15克，大鸭梨1个，冰糖少许。将杏仁去皮捣碎，鸭梨去核切块，与石苇、车前草加水同煮，熟后加冰糖，代茶饮。有泻肺火，利水道功效。<BR><BR>　　4、利尿黄瓜汤：黄瓜1个，瞿麦10克，味精、盐、香油适量。先煎芦蓠、瞿麦，去渣取汁，再重煮沸后加入黄瓜片，再加调料，待温食用。有利水道之功效。<BR><BR>　　5、南瓜子：做零食，平时多食。<BR>

                 (一)生活护理<BR><BR>　　1．保持乐观情绪，坚持体育锻炼，减少局部血液淤滞。<BR>　　2．不要憋尿，一有尿意应立即排出。<BR>　　3．保持大使通畅。<BR>　　4．不宜久坐和长时间骑自行车，以免前列腺部血流不畅。<BR>　　5．性生活不宜过度频繁。<BR>　　6. 衣着要暖和，避免着凉感冒。<BR>　　7．忌服阿托品一类药物，以免发生急性尿潴留。<BR><BR>　　(二)饮食疗法<BR><BR>　　1．饮食上注意少食甜、酸、辛辣食品，多食蔬菜、大豆制品及粗粮，适量食用鸡蛋、牛肉、种子类食物如核桃、南瓜子、葵花子等。<BR>　　2．若膀胱有热，尿道涩痛，可饮用绿豆场或食绿豆粥。亦可用黑木耳煎汤服或凉拌黑木耳食之。<BR>　　3．豆瓣酱是降低前列腺增生症及肠癌发病率的良药，食之益。<BR>　　4．禁饮酒。<BR>

                 <P><FONT face=Verdana>　　一、防止受寒 秋末至初春，天气变化无常，寒冷往往会使病情加重。因此，患者一定注意防寒，预防感冒和上呼吸道感染等。</FONT></P>
<P><FONT face=Verdana>　　二、绝对忌酒 饮酒可使前列腺及膀胱颈充血水肿而诱发尿潴留。 </FONT></P>
<P><FONT face=Verdana>　　三、少食辛辣 辛辣刺激性食品，既可导致性器官充血，又会使痔疮、便秘症状加重，压迫前列腺，加重排尿困难。</FONT></P>
<P><FONT face=Verdana>　　四、不可憋尿 憋尿会造成膀胱过度充盈，使膀胱逼尿肌张力减弱，排尿发生困难，容易诱发急性尿潴留，因此，一定要做到有尿就排。 </FONT></P>
<P><FONT face=Verdana>　　五、不可过劳 过度劳累会耗伤中气，中气不足会造成排尿无力，容易引起尿潴留。 </FONT></P>
<P><FONT face=Verdana>　　六、避免久坐 经常久坐会加重痔疮等病，又易使会阴部充血，引起排尿困难。经常参加文体活动及气功锻炼等，有助于减轻症状。 </FONT></P>
<P><FONT face=Verdana>　　七、适量饮水 饮水过少不但会引起脱水，也不利排尿对尿路的冲洗作用，还容易导致尿液浓缩而形成不溶石。故除夜间适当减少饮水，以免睡后膀胱过度充盈，白天应多饮水。 </FONT></P>
<P><FONT face=Verdana>　　八、慎用药物 有些药物可加重排尿困难，剂量大时可引起急性尿潴留，其中主要有阿托品、颠茄片及麻黄素片、异丙基肾上腺素等。近年来又发现钙阻滞剂和异博定，能促进泌乳素分泌，并可减弱逼尿肌的收缩力，加重排尿困难，故宜慎用或最好<BR>不用某些药物。 </FONT></P>
<P><FONT face=Verdana>　　九、及时治疗 应及时、彻底治疗前列腺炎、膀胱炎与尿道结石症等。 </FONT></P>
<P><FONT face=Verdana>　　十、按摩小腹 按摩小腹，点压脐下气海关元等穴，有利于膀胱功能恢复。小便后稍加压力按摩，可促进膀胱排空，减少残余液。 <BR>　　值得提醒的是，本症发展缓慢，病程长，若能从中年开始预防效果更好，除采取上述措施外，还应防止性生活过度，尤其要警惕性交中断和手淫的行为。据临床观察，多数患者只要能坚持自我保健措施落实和注意及时治疗，效果均很好。 </FONT></P>
